All Evansville Vanderburgh Public Library locations have federal & Indiana forms and instructions.  Forms not in stock can be photocopied from our reproducible forms books or downloaded on our public computers.  We also have master copies of the most popular federal publications that you can read in the library.

INDIANA STATE

• Forms, Publications, Information  Visit the Indiana Department of Revenue official web site.
• Indiana's "I-File"  I-File is the Indiana Department of Revenue's free Web-based tax filing service.
  
• The Indiana Department of Revenue Evansville District Office is located at 500 S. Green River Road Suite 202 (Goodwill Building) and their phone number is 812-479-9261. Their hours are 8 am-4:30 pm Monday through Friday. They can provide many Indiana tax forms as well as answer questions by phone and in person. They will also complete state returns for those who bring in a completed federal form – this is available between 9 a.m. and 3 p.m.
