The Community Resource Guide is a tool to help members of the community locate organizations that can assist with a variety of services ranging from shelter to food and even medical resources. The listing below is divided into the following categories: Long-Term Housing, Immediate Shelter, Food Pantries, Medical, Employment, and Legal. Each resource will have its address, phone number, linked website, brief description, and any known documents that may be needed to use the services they provide. 

Near the bottom of the page, you will find a list of commonly needed documents/forms and ways to possibly obtain them or replacements.

Commonly Needed Forms/Documents

How to obtain commonly needed documents and forms

Driver’s License | Replacement | Photo ID

  • Please note: You may only replace a driver’s license, learner’s permit, CDL, or identification card a maximum of 10 times online before you must visit a BMV branch.
  • To replace online, applicant must create a myBMV account by providing their driver’s license number, last four digits of their social security number, and legal ZIP code.
  • Some applicants are not eligible to renew or replace online because their residential address, township, or zip code has changed, or they amended some other information with the BMV after their last card was issued. If you need to change your name or residential address, you will need to visit a branch to amend your license.

Indiana Birth or Death Certificate

  • Certified copies of birth and death certificates are available from the Indiana Department of Health (IDOH) online with payment using a credit card through the Vital Records Online Ordering System. If you prefer to pay by check or money order, please submit the request via mail.
  • You may also obtain a copy by visiting the Local Health Department where the birth or death occurred. (Check here to find the county office locations). IDOH does not provide on-site service. For additional information or questions contact IDOH Vital Records at (317) 233-2700.
  • For American Citizens born outside of the U.S., you will need to contact Bureau of Consulate Affairs.

Benefit Verification Letter

The Benefit Verification Letter, sometimes called a “budget letter,” a “benefits letter,” a “proof of income letter,” or a “proof of award letter” serves as proof that you either:

  • Get Social Security benefits, Supplemental Security Income (SSI) or Medicare.
  • Have never received benefits or SSI.
  • Have applied for benefits.

You can request one online by using your Social Security account, which will allow you to immediately view, print, and save a copy of the letter. You can call to request one at 1-800-772-1213 (TTY 1-800-325-0778), Monday through Friday from 8:00 a.m. to 7:00 p.m.

Pay Stubs

Through your employer which should be able to direct you to the appropriate website/portal. The Library can assist by providing internet access and limited guidance. You will need to know how to log in (i.e. login information such as username and password).

  • Supportive Services for Veteran Families (SSVF) helps and supports low-income veterans who are homeless, at risk of homelessness and/or need help transitioning to permanent housing. Will need a Military ID Number.

Homeowners are selected based on their need for housing, ability to repay a zero-percent APR mortgage, and willingness to partner with Habitat. Because of Habitat’s no-profit loans, the homes are built on volunteer labor, and mortgage payments are affordable for low-income partners.

The resources below marked with * are part of the Evansville Emergency Food Pantry Consortium (EEFPC). When new to EEFPC, you will need social security cards and birth dates for people in household. Once in the system, you will just need photo ID. Individuals can visit once every 30 days to an EEFPC member for food.

Hangers by EVSC Foundation

  • Hangers serve any EVSC student in need, with a recommendation from a school counselor, principal, social worker, nurse, or other school staff. An appointment is scheduled once a recommendation is received.
